Position Description

Join our team as a Full-Time Community Health RN!

UPMC Children's Hospital of Pittsburgh is hiring a Community Health RN to work primarily at our Lawrenceville Family Care Connection site, as well as, in clients' homes and one day per week at the Mount Oliver Primary Care Clinic. This position will work Monday through Friday, 8:30am to 5:00pm, with occasional evening shifts required.

The Community Health RN provides support services to teachers, parents, children at-risk and others in various community settings. Services include care coordination, general classroom consultation, modeling of healthcare practices, in-service training, acting as a liaison with primary care physicians, and providing direct care to children.

Primary Responsibilities:

  • Identify health needs and stresses prevention and provides guidance on growth and development.
  • Provide appropriate referral and follow-up.
  • Accept consultation from pediatrician on the primary care team.
  • Collaborate with an interdisciplinary team of professionals and parents to identify individual health needs, and write and implement Individualized Developmental Healthcare.
  • Provide education on health-related topics as requested by community groups, parents, and other partners.
  • When working in the Family Care Connection (FCC) Department, will provide pre and post-natal medical home supervision and referral support.
  • Plan of goals and strategies for children with medical conditions. (FCC nurse also works on family goal plans).
  • Conduct formal and informal (on-the-spot) in-service training and mentoring of teachers and childcare staff regarding medical and developmental issues of children at-risk.
  • Provide teaching and consultation to teachers and others in group settings regarding general healthcare promotion and prevention techniques (i.e., universal health precautions).
  • Provide direct teaching and service to children, teachers, and parents in classrooms and in community settings by modeling individualized medical and healthcare procedures. (When working in the FCC, maintains hands-on pediatric skills with a bi-annual rotation through a Primary Care Center).
  • Liaison with the child's primary care physician for feedback and approval of the healthcare plan.
  • Offer healthcare representation on school and agency health advisory groups that develop policies for children with medical needs.
  • Assist in conducting research on healthcare outcomes in early childhood settings.
  • Assist in writing grants and gaining interagency contract.
  • Registered Nurse license required
  • BSN preferred
  • 3 years of experience in community health, public health, school, or balanced clinical and non-traditional settings

Licensure, Certifications, and Clearances:

  • Driver's License
  • Registered Nurse (RN)
  • Clearances with Renewal:
  • Act 31, 33, 34, & 73
  • Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
